To print a copy, please RIGHT CLICK on the Records Release Download button below.  LEFT CLICK "Save Target As" (or "Save Link As" in a Windows-based PC whereas if you have a Mac Computer, the options are "Down Link File As" or "Down Link File) and designate a location on your computer (typical options are "Desktop" or "My Documents"). 

Go to that location to find the file "FEMME VITALE RECORDS RELEASE FORM.doc", open it and print off a copy.
