Change and transition are seldom easy, but are inevitable. After more than 35 years providing obstetric and gynecologic care, the most recent 25 years here in SW Michigan, I closed my medical practice as of December 15, 2011. My wife Susan and I want to thank you for giving us the opportunity to serve you, your friends and family.
All charts were closed on December 21, 2011 and nothing more may be added by Dr. Cairns or his staff. If you schedule your mammogram, designate the provider who you wish to receive the report.
We will continue to address questions or concerns you may have regarding claims, statements, billing, unpaid balances, insurance payments, denials and refunds for approximately two months. Please contact us promptly by leaving a message at our regular phone number, 269-982-3366 with your name and at least one number we may use to reach you.
For those who do not already have another provider, we are recommending Drs. DeAnn Kutzner, Jin Jyung, Rachel Proctor and Heidi Grabemeyer-Layman at South Shore Women’s Health Care, 2690 S. Cleveland Avenue, St. Joseph, MI 49085 (269) 428-2800 FAX (269) 428-7177.
South Shore Women’s Health has graciously agreed to be the Legal Custodian of Femme Vitale charts for the next ten years beginning January 1, 2012 and so ensure the privacy and accessibility of your medical records in a separate, password-protected storage system accessible only by a very limited number of HIPAA-trained individuals.
A signed Records Release is required for transfer of records from Femme Vitale storage to any medical provider including South Shore Women's Health Care. Upon receipt of a signed Records Release at Femme Vitale Records Custodian, SSWHC, 2690 S. Cleveland Avenue, St. Joseph, MI 49085 (269) 428-2800 FAX (269) 428-7177, after January 1, 2012, South Shore will import your records to their own electronic medical records system or transfer your records to other providers as requested at no charge. Per standard protocol, you will be charged a minimal $5.00 fee for a personal copy of your record.
You may print a copy of the Medical Records Release form on this website. After completing all sections, please mail or fax the form as directed on the form. Records Release forms will also be available from South Shore Women’s Health after January 1, 2012.
Many patients have thoughtfully inquired about our retirement plans. We are very content to continue to reside in SW Michigan where we are close to our children and grandchildren (we have been blessed!!)
As you might expect, we do not plan to “rest and rust” – we donated our rocking chair to our daughter with the youngest grandbaby (Gabriel Lawrence nonetheless!!!). Our new pursuits will include sponsoring small groups for people interested in becoming debt-free using the “War On Debt”™ program and helping families with children “Grooming The Next Generation For Success”™ program – both very powerful courses available from www.DaniJohnson.com. We have observed the results of these materials and are very impressed!
Through Dani Johnson, we have obtained the training and skills to help families become debt-free and raise children who think it’s cool to be respectful and honoring to their parents, siblings, teachers, authorities (eventually a spouse) and to make a positive impact on their world. We are setting up the next WOD and GTNGFS small groups to start shortly.
We are also actively interviewing self-motivated, dependable, team player-type people who are absolutely serious about building a long-term business that is willable, sellable and trustable. This is not for the faint-of-heart but for anyone over 18 who wants to be trained by the best to make a very respectable income.
I hope that it will come as no surprise to you that we will also continue to minister to people’s spiritual dimension as we have in the past. It has been our daily prayer that we might touch your life, in the office and hospital, with the unconditional love of Jesus. We have tried to live, practice medicine and give to all our patients as we have received from Him. There will, most certainly, be opportunities to help people through mission trips in the US and overseas.
